Vasectomy vs tubal ligation
Both are permanent contraception and both work more than 99% of the time. The difference is everything around that: a vasectomy is a 15-30 minute local-anaesthetic procedure costing $410-$620 out of pocket, while tubal ligation is abdominal surgery under general anaesthetic with a hospital admission and a bill that typically runs $1,500-$6,000. Here’s the full side-by-side.
| Vasectomy | Tubal ligation | |
|---|---|---|
| What’s done | The vas deferens (sperm tubes) are cut and sealed through a tiny puncture or incision in the scrotum | The fallopian tubes are clipped, cut or removed, usually via keyhole surgery through the abdomen |
| Anaesthetic | Local anaesthetic is standard; you’re awake. Light sedation is optional at some clinics | General anaesthetic - you’re fully asleep, with the added risks that carries |
| Where & how long | Clinic room or day surgery, 15-30 minutes, home the same hour | Operating theatre with hospital admission; usually home the same day, sometimes overnight |
| Typical cost (Aug 2026) | $410-$620 out of pocket after the $233.90 Medicare rebate (checked Aug 2026) | Roughly $1,500-$6,000 privately once surgeon, anaesthetist and hospital fees are counted (checked Aug 2026); free in a public hospital but with a long wait |
| Recovery | Desk work in 1-2 days; heavy lifting after 5-7 days | Most women need about a week off, with several weeks before full activity |
| Effectiveness | Over 99% - about 1 in 2,000 failures after a clear semen test. Not immediate: contraception is needed until the ~12-week test | Over 99% in the first year - long-term studies found roughly 1 in 200 pregnancies over 10 years. Effective immediately |
| Reversibility | Difficult and expensive - $5,000-$15,000+ with success falling over time. Treat as permanent | Also difficult and expensive, and impossible if the tubes were removed. Treat as permanent |
On safety, the gap comes down to anaesthetic and access. A vasectomy happens under local anaesthetic through skin that’s millimetres from the target; most men report brief discomfort and are back at a desk within two days. Tubal ligation means a general anaesthetic and instruments entering the abdomen - a well-established operation, but a materially bigger one, with complication rates to match.
On cost, the asymmetry is even larger. A private vasectomy leaves most men $410-$620 out of pocket after the Medicare rebate (checked Aug 2026). Private tubal ligation involves three sets of fees - surgeon, anaesthetist and hospital - and typically lands between $1,500 and $6,000 depending on insurance and where it’s done. The public system offers both for free with a Medicare card, but waits of 6-12 months are common, and Queensland public hospitals generally don’t offer vasectomy at all.
One thing tubal ligation does better: it works immediately. A vasectomy isn’t effective until a semen test at about 12 weeks confirms clearance, so you’ll need other contraception until then.
Purely on the medicine, a vasectomy is the simpler and cheaper of the two: local rather than general anaesthetic, a clinic visit rather than a hospital admission, days rather than weeks of recovery, for a fraction of the price. That’s why guidelines around the world describe it as the lower-risk sterilisation option for a couple. But this is a couple’s decision, not an accounting exercise - the right answer depends on whose body, whose health history, and who is certain. If either of you is unsure, talk it through properly before booking anything.
